Margot Kushel, MD, director of the Benioff Homelessness and Housing Initiative, delivered the Harvard Joint Center for Housing Studies’ 23rd John T. Dunlop Lecture. Dr. Kushel spoke about the impact that homelessness has on an individual’s quality of life, highlighting the lived experiences of unhoused Californians who participated in the California Statewide Study of People Experiencing Homelessness. After the lecture there was a panel, featuring, Boston Healthcare for the Homeless founder, Harvard Medical School professor James J. O’Connell, and Peggy Bailey, a leader of a housing policy research center. JCHS Managing Director Chris Herbert, the moderated the panel. Dr. Kushel said, "While it is really easy to blame people impacted by homelessness, doing so not only is not fair or right, but it also isn’t going to get us any closer to a solution."

Black Californians make up only 7% of the states population, but make up more than 25% of the state's population of people who experience homelessness. A recent special report released by the UCSF Benioff Homelessness and Housing Initiative on Black Californians' experiences homelessness highlights the systemic racism. Kara Young Ponder, PhD, director of community engagement and racial justice at the UCSF Benioff Homelessness and Housing Initiative, said, "California is one million units short of affordable and available housing for extremely low-income renters, of which Black Californians are over represented."

This report reviews who experiences homelessness in the Black community, what may have precipitated their homelessness, experiences during their episode of homelessness, and what impacts their return to permanent housing. These findings aim to inform evidence-based solutions for preventing and ending homelessness for Black Californians.

A special report on the relationship between intimate partner violence and homelessness in California by the UCSF Benioff Homelessness and Housing Initiative, found that 1 in 5 women who become homeless in California flee their homes to escape violence. Participants described the need to plan their exit in advance and to accumulate resource; however, of those who experienced intimate partner violence in the 6 months prior to homelessness, only 12% sought help from a domestic violence organization. Margot Kushel, MD, director of the UCSF Benioff Homelessness and Housing Initiative, said, "Some people honestly had no idea what is a domestic violence shelter. Other people told us when they had to run, they had to run. They didn’t have time to figure out where to find a shelter."

People experiencing homelessness are at an increased danger of dying. A recent study by the National Bureau of Economic Research showed a 40-year-old person experiencing homelessness has a similar risk of death as a 60-year-old person with housing. Previous studies have also found that people who experience homelessness have a greater mortality risk. Margot Kushel, MD, director of the UCSF Benioff Homelessness and Housing Initiative, said, "We’re letting people die because we’re not fixing the underlying crisis."

This paper describes the qualitative component of a large mixed method study, the California Statewide Study of People Experiencing Homelessness. The qualitative study consisted of seven sub-studies across eight counties, each exploring different dimensions of homelessness. BHHI researchers recruited participants from the overall sample, a statewide representative sample of adults experiencing homelessness recruited in eight counties, based on their responses to questions from the survey interviews. The researchers' mereological study design presents an approach for developing complex qualitative policy studies across a range of topic areas.

It’s a common belief about homeless people in California: they move here from other places because of the state’s tolerance, mild weather, and generous services. But according to a recent UCSF study, at least 90% of those experiencing homelessness lived in California before losing their housing and 75% are in the same county they used to live in. In the largest study of its kind in nearly three decades, researchers talked to thousands of participants about their experiences to find out what factors pushed them over the edge into homelessness. The real story, researchers found, has more to do with the state’s poverty, inequality, and high housing costs. Talking about the study findings, UCSF BHHI Director and study author Margot Kushel, MD, and BHHI LEAB representatives, Jesica Giannola and Robynne Rose-Haymer, were guests on KQED Forum's ongoing series about homelessness in the Bay Area.

The UCSF Benioff Homelessness and Housing Initiative conducted an ambitious study, where researchers sought to reflect the experiences of all people aged 18 and older experiencing homelessness in California. The study's findings provide evidence that the lack of housing services is a meaningful barrier to health care and income benefits. "I was really surprised by how little people thought it would have taken to prevent their homelessness. Do we know if people are overly optimistic? Sure, they might be. But I sort of believe people are experts in their own lives and people really felt that, had they interrupted that cycle, they could have hung on, but once they became homeless everything else fell apart." said Margot Kushel, MD, the the study's principal investigator and director of UCSF’s Benioff Homelessness and Housing Initiative.

The California Statewide Study of People Experiencing Homelessness “paints a representative picture” of homelessness and suggests policies backed by evidence, said Dr. Tiana Moore, the Policy Director at the UCSF Benioff Homelessness and Housing Initiative. “There's a lot of noise in California around who's experiencing homelessness and why,” Dr. Moore said, adding that the study pushes back against narratives that are dominating conversations right now. The study found that nine out of ten participants lost their last housing in the state. It is the largest study of its kind since the mid-1990s and the first large-scale study to use surveys as well as in-depth interviews, allowing researchers to “amplify the voices of those who are experiencing homelessness directly,” Dr. Moore said.

California’s homelessness crisis is a homegrown problem that is deepening amid a shortage of affordable housing and emergency shelter, and it’s often the brutal conditions of living on the street that trigger behavioral health problems, such as depression and anxiety, researchers found in a comprehensive study on homelessness. The new findings show that at least 90% of adults who are experiencing homelessness in the state became homeless while living in California.

With modern home prices out of reach for many California residents on fixed incomes, older adults have become the fastest-growing segment of the unhoused population across the state, according to new research from UCSF. California makes up 30% of the nation’s unhoused population and is home to half of all the country’s unsheltered population, according to the landmark study, which looks at how people in the Golden State become unhoused, experience homelessness, and exit homelessness. Nearly half of all unhoused adults in the study were age 50 or older, and Black and Native American residents were “dramatically overrepresented,” the study shows.
“The results of the study confirm that far too many Californians experience homelessness because they cannot afford housing,” said Margot Kushel, MD, director of the UCSF Benioff Homelessness and Housing Initiative and principal investigator of the landmark study.

A new study published by the University of California, San Francisco, paints a different picture, one of people who were working and living in poverty in the state until they suddenly lost their homes. Not knowing where to turn, they ended up on the street, where they endure violence and poor health as they try for years to climb back to stability.
“Something goes wrong, and then everything else falls apart,” said the study’s lead researcher, Margot Kushel, MD, director of the UCSF Benioff Homelessness and Housing Initiative. “Everything in their life gets worse when they lose their housing: their health, their mental health, their substance use.”
