People who live in cars and RVs throughout the Bay Area are moving between cities as crackdowns on street encampments ramp up, a trend that pushed Oakland officials to adopt a new policy speeding up tows over concerns the city is becoming a “destination” for displaced residents. BHHI Director Dr. Margot Kushel says that cities are competing in a “race to the bottom” to pass stronger restrictions on encampments than their neighbors.

Over 60% of permanent supportive housing (PSH) residents smoke cigarettes, and over 50% use cannabis. Co-use of tobacco and cannabis may be linked with other substance use. We explored attitudes toward tobacco-free and cannabis-free policies among PSH residents who smoked cigarettes, co-used tobacco and cannabis, or other substances.

In January 2025, the City of Berkeley launched an effort to resolve a longstanding encampment at 2nd and Cedar Street, where most residents lived in oversized vehicles (primarily RVs). The City offered to buy oversized vehicles from residents and provided low-barrier non-congregate shelter with enhanced case management. Residents could choose to participate in one part of the program or both. At the City’s request, the UCSF Benioff Homelessness and Housing Initiative (BHHI) is conducting a scientifically rigorous, independent evaluation to understand what works, what doesn’t, and how future programs can better support people as they move from encampments into shelter and housing.

The number of Latino/a people experiencing homelessness in the United States is increasing due to increased housing costs, economic disruption from the COVID-19 pandemic, and increased homelessness among migrants. There is little known about the lived experiences of Latino/a individuals experiencing homelessness. We conducted qualitative interviews in English and Spanish with 84 participants who self-identified as Latino/a as part of a large mixed methods representative study of homelessness. We analyzed qualitative interview transcripts from the California Statewide Study of People Experiencing Homelessness (CASPEH) to elucidate factors contributing to increased risk of homelessness for the Latino/a population, the vulnerabilities Latino/a individuals face while navigating homelessness, and how these vulnerabilities influence access to housing and services.

Dr. Kushel shares compelling insights from her three decades of clinical practice and research, revealing how the lack of affordable housing creates impossible situations for healthcare providers trying to treat patients experiencing homelessness. From managing diabetes in a tent to storing insulin without refrigeration, she illustrates why "there is no medicine as powerful as housing."

Tobacco use remains the leading cause of preventable deaths worldwide. Despite efforts by many countries to reduce the impact of tobacco products and lower smoking rates, smoking prevalence is higher among people experiencing homelessness. The reasons are complex and go beyond individual choices, including limited awareness about quitting, restricted access to services, and previous negative experiences. Using the SEM framework, which emphasizes the policy and environmental contexts, this study offers a deeper understanding of the context of the lives women experiencing homelessness. The evidence from this study supports the implementation of gender- and age-appropriate holistic approaches to healthcare and tobacco interventions tailored to this community.

People experiencing homelessness in California face high rates of chronic illness and disability, and numerous barriers to care. These health challenges make it harder for people to obtain and retain housing, and homelessness worsens health, trapping people in a vicious cycle. As the homeless population ages, these burdens deepen, with profound implications for individuals, communities, and the systems that serve them. The U.S. overdose crisis is now driven by polysubstance use involving fentanyl and stimulants. We received funding through the National Institutes on Drug Abuse’s Helping to End Addiction Long-Term (HEAL) initiative to conduct longitudinal qualitative research on polysubstance use among people experiencing homelessness (PEH) in San Francisco, California. This paper discusses the rationale for these combined approaches and lessons learned from conducting longitudinal qualitative research with a community in real-time during enhanced risk for criminalization and overdose fatality.
